Donation Request Form

Slice Pizzeria is honored to be asked and proud to be able to participate in community activities; however, we are unable to fund 100% of the requests made. Please fill out the following form (must be submitted at a minimum of 30 days prior to the event) with any and all pertinent information and we will do our best to get back to you in a timely manner.
